YOUR FIRST VISIT           

 

You will be seen by the pain specialist who will perform an extensive evaluation and physical exam. The doctor will explain his findings and recommendations with you. This visit should last more than one hour and up two.


FOLLOW UP VISITS

 

You will be seen by the pain specialist and likely another member of the team. We will review your progess with you. A follow up exam will be performed and new recommendations made.

The Pain Management Center is not a substitute for your primary care physician. Rather, we encourage you to continue with your primary care provider for overall medical treatment.
 

RECORDS

 

Your medical records are confidential. If you wish to have copies of your office records released to someone outside of our practice, you must send us a written request authorizing us to release the records. Please keep out office informed of any change in your name, address, employer, telephone numbers and health insurance coverage. All your records are kept confidential and on computer only, no physical backup or paper files are made.
 

TELEPHONE CALLS

 

Telephone calls will be taken Monday through Friday from 9:00 am to 4:00 pm. If you should call our office and get voice mail, please leave a message and someone will call you back as soon as possible. If you call after these hours or on weekends you will be directed to our answering service. After hours, Doctors will be paged for emergency situations only. It is our policy that we do not refill prescriptions on weekends or nights. If your emergency is such, you will need to go to the nearest emergency room.
